Attention All Coaching Staff

All Volunteers are required undergo the "Police Reference Check Program", To be used only by the Agency to determine the suitabilty of successful, candidates for employment and/or volunteer duties (including Agency board members and contact members) where individuals will have direct contact with children or vulnerable persons.

When you are ready to submit the Waiver, form download the Reference Check file.

When you print the form make sure you print it on LEGAL SIZE paper, you may have to change your print settings. The Police will only accept to completed form on LEGAL SIZE paper.

Have each volunteer complete the first section with personal information and the last section with Applicant signature, witness and date.

Send the completed form, 1st and last sections, along with a cheque in the amount of $15.75 made out to The Toronto Colts, c/o Nick Manzo. Nick will complete the form and forward to the Toronto Police Services for processing.

This procedure will save time and money for all teams.
